Convenient Once-Daily Dosing

Recommended dosing1

  • Initiate Effient® (prasugrel) as a single 60-mg oral loading dose (six 10-mg tablets)
  • Continue at 10 mg, one tablet once daily
  • Patients taking Effient should also take aspirin (75 mg to 325 mg) daily
  • Effient may be administered with or without food

Dosing in low-weight patients1

  • Consider lowering the maintenance dose to 5 mg in patients <60 kg. The effectiveness and safety of the 5-mg dose have not been prospectively studied

Treatment considerations1

  • Effient plus ASA and Plavix® (clopidogrel bisulfate) plus ASA were not administered to UA/NSTEMI patients until coronary anatomy was established in the TRITON-TIMI 38 clinical trial. Because the large majority of patients are managed without CABG, treatment can be considered before determining the coronary anatomy if the need for CABG is considered unlikely
    • For the small fraction of patients who required urgent CABG after treatment with Effient, the risk of significant bleeding was substantial. The advantages of earlier treatment with Effient must be balanced against the increased rate of bleeding in patients who need to undergo urgent CABG

Study considerations related to timing of Plavix loading dose1

  • In TRITON-TIMI 38, the loading dose of Plavix was delayed relative to the placebo-controlled trials that supported its approval for ACS

SELECTED SAFETY: SIGNIFICANT BLEEDING RISK

Effient can cause significant, sometimes fatal, bleeding. In patients ≥75 years of age, Effient is generally not recommended, because of the increased risk of fatal and intracranial bleeding and uncertain benefit, except in high-risk situations (diabetes or prior MI) where its effect appears to be greater and its use may be considered.
